- What does the ADIPEX-P® PerformanceScript®
card offer?
The card offers a special discounted price on a valid prescription for ADIPEX-P®.
- What is the discounted price?
The discounted price is based on the quantity prescribed. A prescription for 30 tablets will cost the patient $46.10. (Price is subject to change at any time.)
- Why do the cards have different group numbers?
The group number is for internal use only.
- What pharmacies will accept the PerformanceScript®
card?
The card can be used at any pharmacy. Pharmacists with questions concerning this program should contact the AdvancePCS™ help desk at (800) 345-5413.
- Can the card be used more than once?
Yes, the same card can be used for additional prescriptions for ADIPEX-P®. After processing the claim, the pharmacist should return the card to the customer for future use. The card is not valid after the expiration date located in the bottom right hand corner of the card has passed.
- How is the pharmacy reimbursed?
Pharmacies are reimbursed every 2 weeks by AdvancePCS™. Pharmacists with questions regarding their reimbursement should call (800) 345-5413 and speak to an AdvancePCS™ representative.

Pharmacists with questions concerning this program should contact the AdvancePCS™ help desk at (800) 345-5413.
Physicians with questions concerning this program should contact their GATE Pharmaceuticals Sales Representative at (800) 292-GATE.
- Where can I get a PerformanceScript® card?
The ADIPEX-P® PerformanceScript® card is supplied by the physician who prescribes ADIPEX-P®. Physicians receive the cards from their GATE Pharmaceuticals Sales Representative. If the physician does not have this card or needs information on the program, he or she may contact GATE Pharmaceuticals at (800) 292-GATE to obtain this information.
